Learn what transition planning is, when it starts, and what to ask for at the IEP table so your student leaves high school with a plan for work, college, or community life.

Presented by Southern Illinois Pathways, a program of SIP Arts Foundation, with Family Matters PTIC, Illinois' Parent Training and Information Center.
