Oasis community health facilitators will be leading "Living a Healthy Life with Chronic Conditions" at Alton Memorial Hospital in room G-252 on the ground floor of the Olin Wing.
Developed by Stanford University's Patient Education Research Center, this self-management course is for those with such chronic conditions as heart disease, arthritis, diabetes, asthma, bronchitis, emphysema and more.
