About the role We are seeking a full-time Home Health R.N. with clinical nursing knowledge, motivation, & strong communication skills. Individuals shall have the independent skills to assess, plan & provide outstanding care to our patients. Works under the supervision of the Home Health Manager. Responsibilities • Completes referrals with physicians’ orders & signatures. Channels referrals appropriately to the Home Health Manager for assignment. • Systematically performs the ongoing interdisciplinary patient assessment & identifies patient needs, including the need for additional health or ancillary services, & admits patients for service when appropriate. • Assesses family &home environment to identify areas needing change, which will enhance patient care & promote safety. • Responsible for the coordination of the healthcare teams to plan for optimal patient care. Documented by writing clear, concise, & accurate clinical notes, utilizing the problem-oriented medical record system promptly. • Documents nursing care plan on a timely basis, makes changes as the patient’s condition warrants & evaluates plan every month. Demonstrates ability to collaborate with the patient, family, & healthcare team members efficiently. • Interprets the admission, discharge, & other policies of the Agency to the patient, physician, family, & other agencies as necessary. Communicates necessary information to the Administrator and/or the Home Health Manager. • Prepares home health aide assignments & written instructions. • Provide skilled nursing care & assessment & services that are ordered by the physician according to the plan of care & promptly revise the plan as necessary in consultation with the physician & other team members as appropriate. • Provides patient & caregiver education & patient, caregiver, & family counseling. • Evaluate & document home health aide utilization at least every fourteen (14) days for skilled care & monthly for Respite care. • Participate in quality improvement programs & assignments. • Other duties as assigned by the Administrator or Home Health Manager. Candidate requirements • Requires knowledge, ethics, & practices related to home health & public health prevention efforts. • Requires rotation with other nursing staff for the on-call system. • Requires effective communication skills & unbiased healthcare ability. • Requires the ability to work evenings & weekends on occasion. • Requires the ability to perform written & oral communication detailing patient care. • Requires R.N. Licensure, up-to-date immunization records, driver’s license, & car insurance.
